Optometry Medical Certificate

Optometrists can use this form to provide applicants with proof of their medical fitness to drive. Included in this simple, one-page optometry medical certificate is room to list identification marks, as well as nine other boxes with questions about the applicant’s visual and auditory abilities based on the optometrist’s opinion, as well as the applicant’s blood type.
