Patient Dental Release

This straight-forward dental release form asks patients for their consent to request and collect information from another dental clinic. Once completed, dental clinics can forward this form to other dentists as proof of authorization to release their particulars to the clinic. Dental can then share important information about the patient’s dental history, including the dates of their last new patient and recall exams, date of last bitewing radiograph, and more.
