Dental clinics can screen patients for COVID-19 symptoms and also collect patient consent with this Canadian Dental COVID-19 Screening & Consent Form. In addition to standard COVID-19 screening questions, the form includes seven additional checkbox symptoms, two patient vulnerability question boxes, and 10 confirmation and consent checkboxes.
