Orthodontic New Patient Intake – Adult

Orthodontic clinics can use this straight-forward new patient form when they welcome adult patients to their practice. Included on the form are boxes asking for basic patient contact information, their dental information, medical history, and their permission and signature. Additionally, patients are asked to list the date of their last dental cleaning, as well as the primary reason for their visit that day.
