Business COVID-19 Screening & Consent

This simple form can be used by businesses to screen for COVID-19 symptoms and collect consent to participate in meetings and other gatherings. In addition to seven yes or no questions, it includes two checkboxes and a signature area to collect participation consent and acknowledgment of risk. The form can be sent to participants digitally for them to complete on their smartphone, computer, or tablet before returning it electronically.
